Your Voice Matters! Take this quick National Survey on Health Reform and Independent Living

The University of Montana Research and Training Center on Disability (RTC:Rural) is teaming up with the Collaborative on Health Reform and Independent Living (CHRIL) to help recruit rural people for a national survey. The National Survey on Health Reform and Disability is an online survey about health insurance and access to health care among people with disabilities. Responses to the survey will provide important information about how people with disabilities access insurance and health care services.
We know that rural Americans currently experience less access to quality health care and lower rates of health insurance coverage than their urban counterparts. Legislative changes at both federal and state levels are likely to impact these rates, particularly for rural people with disabilities. These issues are what makes participation among rural people with disabilities so important. Your voice matters!
The CHRIL survey takes about 20 minutes and can be completed online or over the phone. The survey asks for county and zip code information for making rural and urban comparisons, but no person will be identifiable from their responses. The survey targets individuals between the ages of 18 and 62 who have a disability, chronic health condition, mental illness or other impairment.
National Survey on Health Reform and Disability
The NIDILRR-funded Collaborative on Health Reform and Independent Living (CHRIL) is looking for adults aged 18-62 with disabilities to complete an online survey about getting and using health insurance and health care services.
Why? We want to know how the Affordable Care Act (ACA) is affecting your life. If you have private insurance, insurance from an employer, Medicaid, Medicare, or no insurance please take the survey.
Who? Adults ages 18 to 62 with any type of disability, chronic health condition or mental illness.
How? The survey should take about 20 minutes to complete and your responses are anonymous.
